Prayers for Sparky

My wonderful little dog, Sparky, has been diagnosed with Cushings Disease. I am just heartbroken. :cry:

I know that Cushings is always fatal, but I'd like to pray that my sweetheart pooch can stabilize and stay with us for at least another year or two. He's such a special doggy. He's never hurt anyone or anything, although he ran away from a former owner who obviously hurt him. Spark is patient with our three kitties and with us. He never barks, has never done anything destructive or had accidents (even with the Cushings). He loves affection, walks, playtime and most of all, a few tidbits of "human" food. His sweet face is always there to greet me when I come home and the last thing I see at night is Sparky lying next to the bed.

I wish with all my heart that Sparky will be able to enjoy life for a long while to come, before going to the Rainbow Bridge. He deserves that...

Sue, cushing's patients generally have excellent prognoses, providing appropriate treatment is done and followed up on regularly. It is secondary problems that can occur that make managing difficult.

What is your vet telling you???

Traci, I was going to post at the regular Canine Forum once I got some information from the vet's office. Here's what I was going to post:

Thanks, Traci. We are going to get some boots for Sparky.

My vet wants to be kept well informed about Sparky. She has asked me to call at least once a week concerning how he is doing. Dr. Sharon also asked that I call right away if he becomes very lethargic or anything else significant happens. He will have his next ACTH test in about one month.

His liver enzymes had been very high. After the first treatment of Lysodren, his level had come down to "20". However, my vet wanted his level to be closer to "7", so he has taken two more full doses of Lysodren and will then be taking a dose twice a week for now.

Sparky seems to be pretty much his old self, except he is rather sleepy. Once he's awake, he seems to be fine. Of course, the weakness in his back legs is disturbing. He seems to get around except for slick surfaces.

Traci, my vet told me that Cushings is caused by a brain tumor, which affects the pituitary gland. The pituitary gland makes a huge amount of a hormone called cortisol. This causes great stress on the liver. From what I understand, the Lysodren helps to destroy some of the tumor, greatly slowing down the cortisol production. The liver can then recover. However, the tumor still continues growing (even though it is slow). My vet said we didn't know how long Spark has had this tumor, since he didn't show any of the usual signs. He didn't drink or urinate excessively, or lose a lot of fur. In fact, it wasn't until we gave him a "haircut" on the huge bunch of fur he grew this winter, that we noticed he had a slight "pot belly". So she didn't want to guess how long he would be with us.

The diagnosis was made from the cortisol levels of the ACTH test. There was no ultrasound or any other test done for Cushings. This was all found when we wanted Sparky to have a routine dental appointment and he had the routine blood tests before that. That was when we discovered the high liver enzymes. Sparky does not have diabetes, Thank Heavens!

If you have any hopeful suggestions, I would very much appreciate hearing them. Sparky is around 10 years old, but he has been healthy otherwise and is not a large dog (around 26 pounds). It just isn't fair!! I love him so much, and I can't take another loss soon. (Since last December, my Mom, my aunt, my uncle and my sister-in-law have passed away.) :cry: Sparky is as much a part of my family as the others...

I have to assume by your post that your vet is referring to PD cushings, which is pituitary-dependent. But, without an ultrasound or MRI, I don't know how she is monitoring progression of a supposed "tumor" in the brain, other than gauging response with treatment. Xrays, ultrasound and/or MRI are useful to not only determine tumor activity, but to check other vital organs, their size and shape, function etc. The latter, the MRI, is the standard diagnostic for brain tumors or pituitary tumors. I recommend regular xrays and at least an ultrasound in early diagnosis to monitor other vital organ function because cushings can affect many of them. It could also help define whether or not the cushings is primary or if there is a secondary problem going on. (remember, the heart murmur occured first, for example, but I suspect due to age, and not cushings, unless there is evidence that blood flow is obstructed due to cushings).

The pot-bellied appearance could be due to cushings, but it could also be due to something else, like enlarged liver, enlarged spleen, fluid accumulation, enlarged kidneys or an adrenal tumor (not pituitary tumor)....hasn't your vet addressed this and wanted to persue an ultrasound to rule out secondary conditions?

Don't dismiss diabetes just yet, not only is age a factor but it could become a secondary condition (usually it's the other way around, diabetes and eventually cushings diagnosis), but age, weight loss, stress, urinary tract issues, glucose fluctuations due to cushings effects could bring on diabetes too. I'd suggest testing urine every time you test the ACTH, and a baseline blood profile every 6 months (or sooner if other symptoms develop or worsening symptoms).

I realize there is a significant cost involved for additional diagnostics, but if this were me, I'd want an ultrasound and MRI to definitively diagnose and monitor progression. Sometimes, testing the ACTH and other bloodwork can be helpful in monitoring, but it sounds like your vet is simply using the ACTH levels and not truly defining other potential problems (liver, kidney, heart, or other organs that could be in trouble). I do not want to scare you, but you truly want to know if there are secondary conditions going on undetected.

The good news is that it seems he is responding to the lysodren, and that's good, but again, you want to closely monitor and watch for other vital organ problems.

Traci, I live in "the sticks", and I've been told that there isn't an MRI for critters available anywhere around here. However, I will request X-Rays and an ultrasound. I'm also going to have a new set of blood tests done.

It appears that Cushings is complicated and that secondary problems can be as (or more) important than the Cushings itself. But thank you for giving me some hope that he could still live a happy, healthy life for at least several years yet. My vet made it seem so hopeless :cry: ...
