Yes, urinating is a very good indication that something could be wrong physically and therefore should be taken to vet. However, once a uti or anything medically wrong has been ruled out there are a few things you can look at.

My 18yr old male occasionally urinates on something when his is really stressed about something. Anytime we get ready to move and start packing our things, he starts urinating. Usually on anything sitting on the floor like a flat box or someones clothing. Also, he urinates whenever he gets really stressed about anything.
So my point is that this can be very normal and to me is part of owning a pet. After all, my hubbie puts up with me when I'm in a bad mood too right?

Also, there are other things to consider. I got a litter pan lid and my cat didn't like it so decided to go outside the bos. I removed the lid and he started going in the box again. Perhaps you've changed the litter brand or litterbox location. Perhaps there is another kitty around that he doesn't like to share a box with.

I had a friend who viewed her cat's tendency to be finicky as the cat having attitude or being "stuck up". She tried to rationalize with the cat as if it were human. I said to her; "perhaps a cat was not the right pet choice for you".
Just recently I'd gotten on to him for something and the next day he gotten up on top of my desk and urinated on my large desk calendar! I could have thought he did it just to "piss" me off but after he later urinated on my bathroom throw rug, I took him to the vet to be told he had a severe UTI. He's still on antibiotics.
