OK xrays and urinalysis it is. And, new doctor for his eyes.
He is a big cat 17.5 pounds, but he's big all over (in other words, he's not fat). I've asked about his weight and whether he should be on a diet, and she said that he was at a good weight for his size, so no need for a diet. He's eating Science Diet Mature Adult.
We live in a three story house, so he gets a fair amount of exercise by simply climbing the stairs. I wouldn't know how to encourage additional exercise.
He has a regular cat bed during the day, and sleeps with me at night. Yes, the vet encouraged warmth, so we have a heated blanket.
He has regular vet visits because of the series of problems he's been having, so he is getting regular checkups.
The broken tail occurred right after he went blind. It was our fault, our ceiling was leaking so my husband brought a ladder upstairs into the bedroom to see what was happening, and left the ladder there in case he needed to reinspect the leak. Guido went into the room and knocked the ladder over onto his tail (at least that's what we think happened ... we were at work when it occurred). Luckily, the tail healed. We have since "child proofed" the house.
The vet said the broken toe was most likely from stubbing it ...
I'm not sure how to make the house any safer. We tried keeping him in a room away from the other cats and we removed all the furniture, but he was absolutely miserable. He would just cry. All of the cats now get along, so I don't think they're the culprits, unless the behave completely different when we are not home.

OK, dumb question, how do I find a feline opthamologist? I tried the yellow pages under feline opthamologist and vet opthamologist, and it came up empty. I extended the search to include locations within 100 miles, and still it came up empty...

Your vet should be referring you to one, ask her to refer you. Most clinics, if the vets aren't experienced, will have a veterinary opthamologist referral, who usually rotates his/her schedule and visits local clinics and examines/treats patients.....or, they may have their own clinic...your vet was/is obligated to refer you.
