Not painful, just slightly uncomfortable for a moment.
A fine needle is inserted directly into the bladder, the vet draws enough urine to get a good sample for testing, needle is drawn back out....the entire procedure only takes a few seconds. I've never had a cat yet display obvious signs of pain or discomfort.
Cystocentisis is ideal for urine samples because the urine is uncontaminated (i.e., contamination can occur when urine passes through the urethra and vaginal/penile openings), it is a fresher sample, and easy to obtain if there is a partial or full bladder.
